The Patient Portal is an easy, safe, and secure way to connect with us online. Provide a personal email address to a Zufall staff member and within a few minutes you will receive an email with login details. Enroll today to get access to time-saving features such as:

Appointments

View, schedule, re-schedule, or cancel appointments online. Receive appointment confirmations and reminder notifications.

Visit Summary & Treatment Plans

View the summary, notes, and treatment plans after a medical or dental visit. (Not included: behavioral health and confidential visits)

Lab Reports & Referrals

View the results of labs, imaging studies, and procedures after they are received by Zufall Health. View current referrals.

Prescription Refills

Request refills of medications prescribed by your medical provider before you run out.

There are two ways to connect to the Patient Portal

Connect Using the Website

Give a Zufall staff member your personal email address. You’ll receive an email invitation from Zufall Health shortly. If you don’t see the email invitation in your inbox, check your “junk” folder.
Open the email invitation and click the link
Enter the temporary username and password you received in the email invitation.
Follow the on-screen instructions to set up an account and change your username and password.

Connect Using the Healow App

Log in to the Patient Portal using the website (as described above) at least once BEFORE setting up the healow app.
Search for “healow” on either the App Store or Google Play or click the buttons below.
Download the app
Enter the practice code: HGDGAA
Enter the Username and Password you created
