Service Fees
Zufall Health has a sliding fee discount program to ensure affordable access to all services. Fees are based on services received and can be adjusted based on your income and family size. Fees for medical services typically range from $20 to $80. Fees for dental services typically range from $40 to $200. Patients may have to pay separately for certain lab services, vaccines, and contraceptive supplies.
Patients with insurance will pay the co-pay indicated on their insurance card. Insured patients may also participate in the sliding fee discount program if their co-pay is higher than the fee on the sliding fee discount schedule.
All patients wishing to participate in the sliding fee discount program must provide documentation of income and family size.
No patient will be denied services due to inability to pay.
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical and dental care will cost. Under the law, health care providers need to give patients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
