2024
Zufall Health Receives Three American Heart Association Outpatient Program Achievement Awards

In October 2024, Zufall Health received the following awards from the American Heart Association:

  • BP Gold + Award level recognition, the highest, by the American Heart Association® and the American Medical Association. These awards celebrates physician practices and health systems, who treat patients with hypertension, for achieving blood pressure control rates at or above 70 percent within the populations they serve. These achievements will ultimately lead to a reduction in the number of Americans who suffer heart attacks and strokes.
  • Check. Change. Control. Cholesterol™ Gold Award for commitment to reducing the risk of heart disease and stroke through improved cholesterol management. More than 70 percent of Zufall’s adult, at-risk atherosclerotic cardiovascular disease patients are appropriately managed with Statin therapy.
  • Type 2 Diabetes℠ Gold Award for its commitment to addressing the relationship between diabetes and the risk of heart disease and stroke. The Participant award recognizes practices that have completed data submission, including clinical measure entry, and committed to improving quality of care for patients with type 2 diabetes and cardiovascular risk factors

In August 2024, Zufall received three Community Health Center Quality Recognition (CHQR) Badges from HRSA. These badges recognize health centers that have made notable quality improvement achievements in the areas of quality and health equity. The three badges are:

  • The Health Center Quality Leader (Gold) award, which indicates that Zufall Health’s overall clinical quality measure (CQM) performance ranks in the top 10% nationwide
  • The Access Enhancer award, which is given to health centers who have 1) earned at least one Health Center Quality Leader or National Quality Leader badge or improved by at least 15% in one or more Clinical Quality Measures in back-to-back reporting years; and 2) have at least a 5% an increase in total patients and patients receiving mental health, substance use disorder, vision, dental, or enabling services in back-to-back reporting years.
  • The Health Disparities Reducer award, which is given to health centers with at least a 10% improvement in low birth weight, hypertension control, or uncontrolled diabetes Clinical Quality Measures and have met set targets for measures in all racial and ethnic groups served within the last reporting year. 
Zufall Health Awarded Accreditation from The Joint Commission

Zufall Health announced its first-ever accreditation by The Joint Commission, the nation’s oldest and largest standards-setting and accrediting body in health care. The Joint Commission’s Gold Seal of Approval® for Ambulatory Health Care Accreditation recognizes Zufall’s ongoing commitment to providing safe and quality patient care.

The accreditation follows a rigorous, three-day evaluation of multiple Zufall sites. During the visits, a team of Joint Commission reviewers evaluated compliance with Ambulatory Health Care standards spanning several areas including patient rights, patient satisfaction, emergency preparedness, infection prevention and control, risk management, and quality improvement. The surveyors also conducted onsite observations and interviews.

The Joint Commission’s standards are developed in consultation with health care experts and providers, measurement experts and patients. They are informed by scientific literature and expert consensus to help health care organizations measure, assess and improve performance.

 

2023
HRSA COMMUNITY HEALTH CENTER QUALITY RECOGNITION 2023

In November 2023, Zufall received four Community Health Center Quality Recognition (CHQR) Badges from HRSA. These badges recognize health centers that have made notable quality improvement achievements in the areas of quality, health equity, and health information technology. The four badges are:

  • The Health Center Quality Leader award, which indicates that Zufall Health’s overall clinical quality measure (CQM) performance ranks in the top 10% nationwide

  • The Addressing Social Risk Factors badge, which recognizes health centers that are screening for social risk factors impacting patient health and are increasing access to enabling services. Health centers must meet the following two criteria:
    • Collect data on patient social risk factors
    • Increase the proportion of patients receiving enabling services between consecutive UDS reporting years
National Committee for Quality Assurance Recognition

In 2022, the National Center for Quality Assurance (NCQA) recognized Zufall Health as a Patient-Centered Medical Home for all eligible sites. The award recognizes Zufall’s systematic use of patient-centered, coordinated care management processes.

Zufall recognized as a Healthy People 2030 Champion by the U.S. Department of Health and Human Services

In February 2022, Zufall was recognized as a Healthy People 2030 Champion by the U.S. Department of Health and Human Services, indicative of Zufall’s commitment to work toward achieving Healthy People 2030’s vision of a society in which all people can achieve their full potential for health and well-being across the lifespan.

2021
Ricardo Salcido of HIV Services receives PROUD Award

In December 2021, Zufall’s HIV Services – Prevention and Retention Program Manager, Ricardo Salcido, received a PROUD Award from Robert Wood Johnson University Hospital. The award recognized his tremendous contribution and outstanding commitment to the LGBTQIA+ community in the advancement of their health care needs.

Health Champion of the Year

In December 2021, Zufall was named Health Champion of the Year by the Morris County Hispanic-American Chamber of Commerce.

ZUFALL RECOGNIZED AT THE GOLD + LEVEL FOR COMMITMENT TO BLOOD PRESSURE CONTROL

In October 2021 at the Regional Hypertension Summit, Zufall Health was named one of two health care practices in New Jersey to achieve Gold + level recognition, the highest, by the American Heart Association® and the American Medical Association for Target BP awards. These awards recognize practices for their unwavering commitment to achieving better blood pressure control, with the ultimate goal of reducing the number of Americans who have heart attacks and strokes. Gold + practices demonstrate a commitment to measurement accuracy and reaching 70 percent blood pressure control or higher within their patient population.

HRSA Community Health Center Quality Recognition

In 2021, Zufall Health was awarded a HRSA Community Health Center Quality Recognition (CHQR) Badge for Patient-Centered Medical Home (PCMH), awarded to health centers with PCMH recognition in one or more delivery sites. Zufall also earned Gold tier recognition, placing Zufall among the top 10% health centers in terms of overall clinical quality measure performance, and was honored for COVID-19 Testing, indicating that Zufall tested more than 50% of 2020 patients from April 10, 2020 to July 2, 2021.

2020 Somerset County Economic Vitality Award

Zufall Health was honored with the 2020 Somerset County Economic Vitality Award at the December 2020 Annual Meeting of the Somerset County Business Partnership. The award recognized Zufall as an organization of frontline workers who went above and beyond in their service.

2019
CMA Karen Ponce Receives Healthcare Hero Award

In October 2019, Karen Ponce, Certified Medical Assistant, was presented with a 2019 Healthcare Hero Award from the New Jersey Primary Care Association for her selfless commitment and unwavering dedication to the mission of Zufall Health.

The Kathleen Nordman Dental Quality Award’s First Recipient is Dr. Antonella Maietta

Dr. Antonella Maietta is the first recipient of Zufall Health’s Kathleen Nordman Dental Quality Award, an award given to a Zufall dental provider in recognition of exemplary quality of care and patient engagement. Dr. Maietta has been a valuable member of the dental team starting her career at Zufall in our Flemington Healthy Smile Center and on the mobile unit. Her dedication to mobile dentistry and the van program has helped contribute to the great success of the Delta Dental Mobile. Like many Zufall dental providers, she has forged a special bond with her patients which contributes to their high satisfaction.

Zufall Recognized as a Health Center Quality Leader

On March 11th, Zufall Health received the Health Center Quality Leader recognition from United Healthcare Community Plan. The recognition from United Healthcare Community Plan was to acknowledge Zufall’s Health Center Quality Leader award from HRSA. Quality Leaders are among the top 30 percent of all HRSA-supported health centers that achieved the best overall clinical outcomes, demonstrating high-quality across their clinical operations. Zufall Health was of the seven New Jersey FQHCs to receive the Quality Leader award from HRSA.

2018
Zufall Pediatrician honored by Partners for Health Foundation

Dr. Mario Szuchman, a Zufall Pediatrician, was honored by the Partners for Health Foundation on November 29th with their Community Impact Award in the area of “Access to Health”. To celebrate its 10th anniversary, Partners for Health Foundation awarded 10 Community Impact Awards, and grants totaling $100,000, to recognize nonprofit organizations and individuals making a significant difference in the health and well-being of local communities.

HEAL Case Manager Receives Healthcare Hero Award

In October, Erik Moran, Case Manager of the Health, Education, and Legal (HEAL) Program received a 2018 Healthcare Hero Award from the New Jersey Primary Care Association for his commitment to help low-income Zufall Health patients in Morris County obtain legal assistance focused on issues including housing, income maintenance and domestic violence.

Healthcare Institute of New Jersey Honors Zufall Health

The Healthcare Institute of New Jersey (HINJ) honored Zufall Health with its 2018 Recognition Award for Patient Access and Advocacy at the New Jersey Primary Care Association annual convention in October 2018.

Dr. Rina Ramirez Wins 2018 Samuel U. Rodgers, MD Achievement Award

Dr. Rina Ramirez, Zufall’s Chief Medical Officer, has been named the winner of the National Association of Community Health Center’s ‘s 2018 Samuel U. Rodgers, MD Achievement Award.  Established in 1977 in honor of the late Dr. Rodgers, a health care provider, community activist, and national leader in health and civil rights, this award is presented to a distinguished primary care clinician who exemplifies excellence in clinical practice and leadership at the local, state, or national level.

Zufall President & CEO Presented with 2018 Non-Profit Exemplary Leader Award

On April 19, 2018, the Morris County Chamber of Commerce  presented Zufall Health’s President & CEO Eva Turbiner with the 2018 Non-Profit Exemplary Leader Award. The Chamber’s Non-Profit Excellence Awards honor Morris County organizations and volunteers who demonstrate innovation and leadership that has resulted in new and improved services for clients and has had a meaningful impact on the community.

2017
NIPN Gold Level Achievement in Prevention of HPV-Related Cancers

In December 2017, the National Improvement Partnership Network (NIPN) acknowledged Zufall Health – Dover with  a Gold Level Achievement for our dedication to the prevention of HPV-related cancers through participation in the National Immunization Partnership with the APA’s Learning Collaborative to Improve HPV Immunization Coverage. A Gold Level Achievement is awarded for meeting or exceeding all project goals.

Zufall CEED Program Coordinator receives Healthcare Hero Award

Denise Fallon, Coordinator of the Cancer Education and Early Detection program at Zufall Health in Somerville received the Healthcare Hero Award from the NJ Primary Care Association for her dedication to helping thousands of low-income patients in Somerset County get essential cancer screenings.

Zufall Patient Navigator receives Lester Z. Lieberman Award

On July 17th, Zufall’s Patient Navigator for Seniors, Monica McKenzie received The Healthcare Foundation of New Jersey’s 2017 Lester Z. Lieberman Award for Humanism in Healthcare for her significant efforts in assisting at-risk seniors in achieving desired medical outcomes.

Zufall Pediatrician Named Population Health Hero

In June 2017, Dr. Amy Kotler was named as a Population Health Hero by the NJ Department of Health for her leadership for Zufall Health’s Andale program to combat childhood obesity.

Dental Residency Accreditation

Zufall Health residency program received accreditation from the Commission on Dental Accreditation – Advanced Education in General Dentistry in April 2017.

Best Women in Business Award

In March 2017, Eva Turbiner was named one of the state’s Best 50 Women in Business by NJBIZ Magazine.

CEO Receives National Award

In 2016, Eva Turbiner was honored by the National Association of Community Health Centers with the Elizabeth K. Cooke Most Valuable Player Award for advocacy on behalf of community health.
